Facility Overview
A senior living community is seeking a compassionate and resident-focused Social Worker to support Independent Living residents within a Life Plan environment. This role focuses on wellness, psychosocial support, resident advocacy, and care coordination while promoting a person-centered approach to resident services.

Position Summary / Responsibilities
• Conduct annual wellness visits with Independent Living residents to assess overall well-being and maintain accurate demographic and biographical information
• Meet with residents and family members to discuss in-home care needs and transitions to higher levels of care when appropriate
• Facilitate resident-centered psychosocial support groups and educational community forums
• Lead weekly resident review discussions and provide updates on resident care needs across the continuum of care
• Support resident communication initiatives through online resident portals and community communication channels
• Ensure residents understand grievance procedures and assist with prompt grievance resolution
• Participate in resident services initiatives, activity planning, and resident committee engagement
• Maintain compliance with organizational policies, confidentiality standards, and applicable regulations

Requirements
• Bachelor’s degree in Social Work from an accredited university
• One (1) to three (3) years of experience in a senior living community or equivalent combination of education and experience
• Knowledge of practices within Life Plan or Continuing Care communities
• Strong interpersonal, communication, and organizational skills
• Compassion for older adults and commitment to a person-centered approach
