Physical Therapist — West Reading, PA
Permanent Placement | $50–$55 per hour
Kace Premier Medical Talent is recruiting a Physical Therapist for a skilled nursing rehabilitation opportunity in West Reading, Pennsylvania. Support patients working toward improved mobility, safer movement, and greater independence.
Responsibilities:
• Perform physical therapy evaluations and identify functional limitations.
• Establish measurable goals and individualized treatment plans.
• Provide therapeutic exercise, gait training, balance training, and functional mobility interventions.
• Monitor outcomes and adjust treatment as appropriate.
• Educate patients and caregivers on safe movement and prescribed exercises.
• Coordinate with rehabilitation and nursing colleagues.
• Complete accurate clinical documentation and support discharge planning.
Qualifications:
• Graduate of an accredited physical therapy program.
• Current Pennsylvania PT license, or eligibility to obtain licensure before starting.
• Strong assessment, treatment planning, and documentation skills.
• Ability to work effectively with an interdisciplinary team.
