Position Title: Physical Therapist
Location: Pleasant Hill, CA
Compensation: $60-$65/hour
Schedule: Monday-Friday, 35 hours/week with weekend rotation
Setting: Skilled Nursing Facility (SNF), Rehabilitation, Sub-Acute & Post-Acute Care

Position Summary / Responsibilities
• Provide physical therapy evaluations and treatment plans for patients
• Collaborate with interdisciplinary teams to support patient recovery goals
• Document patient progress and adjust treatment plans as needed
Requirements
• Current Physical Therapist license
• Experience in skilled nursing or rehabilitation settings preferred
• Strong communication and patient care skills
