Advanced Practice Provider (NP / PA) – Primary Care

Position Summary

An established primary care practice is seeking an experienced Nurse Practitioner or Physician Assistant to join its growing team. This is an excellent opportunity for an Advanced Practice Provider who values continuity of care, preventive medicine, and building long-term patient relationships.

Working Monday through Friday with no weekend schedule, you'll manage your own patient panel while collaborating closely with physicians, case managers, and clinical staff to deliver comprehensive outpatient primary care.

Key Responsibilities

  • Perform comprehensive patient evaluations and physical examinations
  • Diagnose and treat acute and chronic medical conditions
  • Develop individualized treatment plans
  • Order and interpret diagnostic testing
  • Coordinate referrals to specialists as needed
  • Manage patient panels and promote continuity of care
  • Participate in quality improvement initiatives and HCC documentation
  • Educate patients and families on preventive care and treatment plans
  • Collaborate with physicians and multidisciplinary care teams

Qualifications

Required

  • MSN, DNP, or graduate of an accredited Physician Assistant program
  • Active California NP or PA license
  • Active DEA registration
  • Minimum two years of outpatient primary care or family medicine experience
  • Strong clinical assessment and communication skills

Preferred

  • Experience with managed care and value-based care models

Compensation & Benefits

  • Salary:$155,000–$170,000 annually
